SEI 700 forms are "Statement of Economic Interest" (SEI) forms that elected and appointed officials fill out to disclose matters that might create financial or economic conflicts of interest. They are due annually from the officials as well as when they enter and leave office. There are multiple "gross flaws" in the system however. For starters, Candidates for office are not required to fill them out prior to the election or inauguration. After inauguration they file their first one, and that's when the people who elected them may find surprises, omissions or other concerns. Does that make any sense at all? And that's just the first of many problems with what should have been a simple oversight system with "chain of custody" on the documents.
Public Service is NOT compatable with Personal Privacy
The candidates and political body claim this lack of disclosures prior to an election is needed for privacy in the event they are not elected. The absurdity there is palpable, and after this, that poor excuse for nefarious control of the election system should disappear. If someone wants privacy they need NOT seek public office.
As a candidate for public office, If you aren't willing to go public on disclosures prior to election we can have no contract to employ you after the election.
What Glenn Church and anyone who supported these filings exposed is a seriously afflicted pathology. At this point some type of sociopathic, psychopathy, bi-polar, dissociate disorder, intentional inversion, cult orientation, or other affliction must be considered.
It's presumed at this time Church believes there is no consequence for incomplete and errant answers, inconsistent answers, "negligence", and patent omissions that are very material.
Ultimately Church himself is responsible and accountable for these forms. He can decide if he wants to take whatever comes or deflect to any support he had preparing and filing the documents.
What you are about to read should be county, state, and country rattling, once this process, the corruption of this process, and ramifcations are understood.
SEI 700 forms are the most critical document elected officials fill out for purposes of transparency and accountability.
It is the only formal promise they make to the voting public about financial and economic interests that might cause conflicts with their ability to make decisions on behalf of constituents.
The term "Verification" or "Verification Block" is a standard legal term that applies to many legal documents.
In the case of the SEI 700 forms, at the bottom of page 1 is a "Verification"/ "Verification Block" (referring to a blocked out part of the document).
Inside a verification block you will typically find 1) a request for personal information 2) an affirmative statement indicating they have engaged properly in filling out the form under penalty of perjery and 3) their signature.
"Is the document verified" or "Is the document properly verified" are two questions someone with legal interest might ask if a document's content was called into question.

Below is an example of a properly completed Verification Block from one of Glenn Church's 2023 SEI 700 filings.
NOTEs:
This block asks the signer to provide 1) Full Mailing address 2) Daytime phone number 3) email address 4) date signed and 5) signature
The Perjury Attestation reads: "I have used all reasonable diligence preparing this statement. I have reviewed this statement and to the best of my knowledge the information contained herein and in any attached schedule is true and complete. I acknowledge this is a public document. I certify under penalty of perjury under the laws of the State of California that the foregoing is true and correct."
There is a note under the Signature Line in very small text that says, "File the originally signed paper statement with your filing office"
If an SEI 700 form has to be ammended, the form to do that actually includes the full Verfication Block on page 1, but each page then also has a verifcation block in the lower right corner of each page.
The Government Official responsible for ensuring documents are complete and properly verfied is called a "Clerk" aka "The Keeper of Public Trust" per Monterey County's own website
While some may think of the term "clerk" as referring to a "secretary" or someone with a "menial task", in the game of Government and Commerce, the Clerk should be thought of more like the Quality Control Referee. The Clerk is the person who weighs in Fighters before a fight to assure they meet weight or they may be the Referee who weighs footballs and checks pressure to make sure no funny business is going on. A County Clerk is such an important job it's an Elected Postion wth reporting only to the public like a DA, Sherrif, Auditor and Treasurer (as seen in organization chart below)
SEI 700 Forms for Supervisor are supposed to be filed with THREE clerks.
The Clerk of the Monterey County Board of Supervisors ( a direct report to the Board's Adminstrative Office)
The Clerk of Monterey County ( an elected official like the Controller, DA, Sheriff and Treasure-Tax Collector)
The Clerk of the FPPC (Fair Poltiical Practices Committee) < the state >
